The Anaheim nursing home at which the choking death occurred had a history of warnings including poorly secured doors. In this most recent event, an elderly man suffering from dementia died after choking on a tuna sandwich. His death was erroneously reported as a heart attack, but an investigation illuminated a different cause. Due to his condition, the man was to be served pureed food only. During the day of his death he was once fed non-pureed food on which he choked but recovered. Later in the day he was left near a food cart, which contained sandwiches. He took one, choked and died. Allegedly, staff at the residential home did not check his airway or provide emergency resuscitation during the incident. The state fined the nursing home $75,000.
