UCLA is being fined $32,000 for the fatal lab explosions that killed a 23-year old research assistant. The lab assistant was burned over 40% over her body when the fire and explosion occurred. She died 18 days after the accident that happened December 2008. The OSHA investigation uncovered three separate violations of CA workplace safety regulations at the university chemistry lab. They were (1) previous problems discovered during an internal safety inspection had not been rectified; (2) the victim was not wearing protective clothing required for lab work; and (3) the victim had not received proper training.
